DECLARATION AND PRIVACY ACT 1993

I/We:

  • Authorise this application to be sent to South Island Vehicle Finance Ltd or their associated finance company, and agree South Island Vehicle Finance Ltd or an associated finance company, may collect from credit reporting agencies, (specifically including Veda Advantage), credit providers, my/our employer/s, accountants/s, the Courts (regarding outstanding fines) or other named person/s or relative/s, such personal, financial and commercial information and reference about me/us as is necessary for the purpose of considering this application and making a credit decision affecting me/us. And I/We agree that you may produce this authority to such parties for the purpose of collecting such information.

I/We acknowledge that if South Island Vehicle Finance Limited / or an associated finance company
provides this finance, they,

  • will keep the personal information during the period the finance is provided and for six months
    after repayment of all credit provided by South Island Vehicle Finance Limited or an associated
    finance company.
  • may collect and monitor personal information about me/us during the term of this loan as is
    necessary for the administration and protection of such credit and decisions regarding ongoing
    credit-worthiness; and
  • may provide any of the personal information to a third party employed by South Island Vehicle Finance Limited or an associated finance company to assist in enforcement of any agreement between us.

Where the personal information may readily be retrieved, I/We shall have access to it and the
right to request correction. I/we apply for the above loan and certify that the particulars supplied
above and overleaf are true & correct, that I/am, we/are over the age of 18 years and are not an undischarged bankrupt.

SUBMISSION OF THIS FORM TO US WILL BE DEEMED TO CONSTITUTE YOUR CERTIFICATION, ACKNOWLEDGEMENT AND AUTHORISATION CONTAINED IN THE ABOVE CLAUSE.
